ELEMENTARY SCHOOL Parent Submit Excuse Form

Please use this form to notify Rockwood Elementary School of your child’s absence and provide any required information. Complete one form per student and submit as soon as possible after an absence occurs.

 

Important Note: Absences of three (3) or more consecutive days may require a doctor’s note. For planned absences, please submit this form in advance whenever possible.

Option 1 – Google Account Users
Use this form to enter the required information and upload a photo, document or PDF of the excuse (doctor’s note, etc.)

Option 2 – Non-Google Account Users
Use this form to enter the required information, then email the excuse as an attachment to Attendance.

Please submit only one form. If you have questions, contact the attendance office. After submitting, you will receive an email confirmation with a copy of your responses for your records.
