Jr / Sr HIGH SCHOOL Parent Submit Excuse Form

Please use this form to notify the Rockwood Area Jr / Sr High School of your child’s absence and provide any required information. Complete one form per student and submit as soon as possible after an absence occurs.

 

Important Notes:  Absences of three (3) or more consecutive days may require a doctor’s note. For planned absences, please submit this form in advance whenever possible.

Option 1 – Google Account Users
Use this form to enter the required information and upload a photo, documnet or PDF of the excuse (doctor’s note, etc.)

Option 2 – Non-Google Account Users
Use this form to enter the required information, then email the excuse as an attachment to attendance@rockwoodschools.org

Please submit only one form. If you have questions, contact the attendance office. After submitting, you will receive an email confirmation with a copy of your responses for your records.
